JOB DESCRIPTION

JOB TITLE: Case Manager

JOB CLASSIFICATION: Exempt Full Time (40 hours)

DEPARTMENT: Catholic Social Services, St. John's Hospice

Housing and Homeless Services Division

REPORTS TO: Social Services Supervisor

DIRECT REPORTS: None

 

SUMMARY:

 

Provide case management/social work services to clients of Saint John's Hospice Residential Program according to social service standards of Catholic Social Services, Saint John's Hospice Mission Statement and the requirements of contracting agencies.

 

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Complete intake paperwork on new clients and orient them to program.
  • Complete full assessment on each assigned client.
  • Develop a comprehensive service plan, based on identified need that includes goals, action steps and appropriate timeframe for completion.
  • Enter all client data into the HMIS and Harmony databases.
  • Meet with all new clients at least once a week, more if necessary during first month of residency. Bi-weekly meetings after the initial intake period.
  • Monitor health, all prescribed client medications and keep med logs updated.
  • Monitor collection and test urine samples
  • Collect and document client participation in the Savings & Fees Program.
  • Monitor progress on service plan goals and objectives. Assist client as needed.
  • Monitor compliance in all treatment programs.
  • Complete housing applications.
  • Maintain case files for all assigned clients.
  • Provide individual counseling within competency limits.
  • Conduct resident group meetings as scheduled.
  • Meet with Case Management Team weekly or as scheduled.
  • Meet with supervisor as scheduled.
  • Interface with outside agencies regarding client care
  • Develop and maintain a comprehensive collection of resources and referral sources available to clients. Refer clients to appropriate resources.
  • Be available to work a flexible work schedule to accommodate occasional various evening and weekend activities.
  • Participate in training opportunities and scheduled meetings.
  • Maintain statistical information regarding program outcomes for regular reports to Catholic Social Services and OSH.
  • Perform other duties as assigned

KNOWLEDGE, SKILLS AND ABILITIES:

  • Computer, and typing skills.
  • Knowledge of homelessness, substance abuse, and mental illness preferred
  • Good written and verbal communication skills

 

EDUCATION:

  • BSW or comparable Bachelor's degree

 

EXPERIENCE:

  • Two years social work experience

 

PHYSICAL REQUIREMENTS:

  • Ability to stand, sit, walk, climb stairs, operate controls, reach out with hands, occasionally lift objects up to 40 lbs.
